13. I have a disability policy through American Fidelity. Who completes the Employer portion of my disability claim form?
Disability insurance is also known as income protection so the employer questions on the disability claim form are payroll questions. The disability claim form should be sent to the Payroll Department after the employee and physician have completed their parts. Payroll will complete the employer section and send the completed form to American Fidelity.
