9. What do I need to do to add my newborn to my health insurance coverage?
To add a newborn the following information is required: a completed health insurance application, a change in status form, a copy of the birth certificate which is given to the mother while in the hospital. We will also need the newborn's Social Security number as soon as it is received. It is possible to add any other children to the coverage when adding the newborn. Remember the newborn must be added within 60 days of the date of birth.
